Levamisole is an often recommended medication for treating parasites, especially camallanus worm. Unfortunately this medication must be dosed low enough to keep the fish safe and it does not always kill the parasites. More often than not it will only paralyze the parasite.

Do as large of a water change as you can without stressing the fish and make sure you do a gravel vacuum if possible to remove baby worms. If your tank is heavily planted, try doing a large trimming so that the medicated water can reach through all areas of the aquarium to effectively medicate the tank.
Treat the tank with the medication. Maintain the water temperature at or near 75°F (23.9 °C). Each 5 gram packet treats 100 gallons of water. So, when you get a 5 gram packet, add 100 ml of water to it and each 1ml of medication will treat 1 gallon of tank water. Any unused medication can be stored in a small bottle that is wrapped with aluminum foil to prevent exposure from light and it can be stored in the fridge. This will last for at least 2 months before your next treatment. It is not mandatory to keep the lights off when treating the tank, however having a little bit of light i.e. room lighting, would be ideal as you can keep on an eye on the fish in case there are any signs of distress. A partial water change may be necessary to avoid losing fish if the fish are in distress. At the high dosage of 5 grams per 100 gallons of water, some fish are too sensitive and this dosing rate could kill them at 80+ °F. Discus keepers are especially sensitive to this high dose and their normal maintenance temperature.

Continue with the regular feedings and don’t do another water change for at least 2 days. Then, on the third or fourth day you can do another major water change. Camallanus Nematodes do not lay eggs. They are live bearers and the flaria are microscopic and easily eaten by fish. However, dead female worms if not removed or eaten by some fish may pose to be a source of infestation. The second dose, usually conducted after 3 to 4 weeks, insures this doesn’t happen. The effectiveness of this medication will be influenced by the concentration of medication used, and the amount of substrate and organics that’s in your tank. This medication is for aquarium use ONLY.
